Why Choose This Plan?

We believe our Comprehensive Dental Plan will help our patients maintain better dental health. It is designed to provide affordability for people who need dental treatment and do not have dental insurance. The standard set forth by the American Dental Association recommends that you see the dentist twice a year. Paying up-front will guarantee that you will return twice a year! Your benefits include a higher discount amount on any additional dental treatment that is needed.

How long am I enrolled?

Activation of the Comprehensive Dental Plan begins on the date when your premium is paid in full. The Plan expires one year later to the date, no exclusions. We will always provide notification of your expiration date, so that you may choose to continue or cancel your benefits.

With your comprehensive plan, there are:

• No yearly maximums

• No pre-authorizations
• No pre-existing conditions
• Immediate eligibility – No co-pays

Program Guidelines

Program Exclusions and Limitations

This program is a membership plan, not a dental insurance plan, and is secondary to any other dental plan.

It cannot be used:

•  In conjunction with another dental plan. 
• For services or injuries covered under workman’s compensation. 
• For treatment which, in sole opinion of treating dentist or doctor that lies outside the realm of their capability. 
• For referrals to specialists. 
• For hospitalization or hospital charges of any kind. 
• For costs of dental care which is covered under automobile medical insurance. 
• Patient’s portion of bill is due the date of service.

This membership plan is only honored by Mark D. Zeigler DMD.  This dental plan is not an insurance plan that can be used at any other dental office.

Do You Know How Much We Care About You?

As part of our dedication to improving the health and lives of others we will screen patients for Sleep-Disordered Breathing (SDB) during your 6-month cleaning appointment.

We will also provide an oral cancer exam during this visit.

Benefit Premiums

Single  $249/yr.
Dual  $498/yr.
Family of 3  $725/yr.
Additional Family $150/yr.
The dual plan is for Parent/Child or Husband/Wife only.

The Family plan includes children who are enrolled as full time students up to age 25 or children who are not full time students until the age of 18.
